Your health and the health care costs are a shared commitment between you and the Company. We offer a range of health care options that balance attractiveness and cost - empowering you to choose the options that are most appropriate and affordable for you.

Flexible Credits
The Company helps make health care more affordable by providing a portion of the cost of your medical, dental, vision, life insurance, and long-term disability in the form of flex credits. In some cases you will receive opt-out credits by waiving coverage that can be used for other benefits.

Medical
We offer three plan designs consistent across all regions.

 • 

HMO/EPO: This option provides health care coverage at negotiated rates through doctors and hospitals that contract with the Company’s network. Other than emergency care, no benefits are provided for services received outside the network. This option utilizes co-pays for certain services.

 • 

PPO: This option also offers a network of providers, but you receive certain benefits whether you receive care in or out of the network. However, you receive higher benefits and have lower out-of-pocket costs with in-network care.

 • 

HRA Plan: This option is designed to empower you to take control of your health, as well as the dollars you spend on your health care. This plan includes a Health Reimbursement Account (HRA) in which the Company places money for you to use for routine medical care. It also includes traditional health coverage, similar to a typical PPO health plan, that protects you against large expenses.

Dental
We offer two dental plans and a Dental HMO where available that cover preventive and basic services. The Enhanced Plan also offers major dental procedures and orthodontia services.

Vision
Taking care of your eyes - whether it's regular eye exams, a fresh set of contact lenses or glasses - is essential to your personal health.

Disability Benefits
Short-Term Disability (STD) provides financial protection if you are unable to work as a result of a covered illness or injury. This benefit is provided at no cost to you.

Programs to Help Improve Your Health
Wellness is a daily commitment to eating right, exercising, and maintaining balance in your life. To create an environment of wellness, all WellPoint facilities are smoke-free effective November 16, 2006. In addition, the Company supports you by offering:

Employee Assistance Program (EAP)
The Company provides EAP to you and your family, whether or not you are enrolled in the Company's medical coverage. Counselors are trained to help you with such issues as marital or family distress, grief counseling, substance abuse and mental illness. This program can also assist you with other issues such as legal assistance, day care and elder care searches.

Onsite Health Services and Screenings
The Company coordinates onsite blood pressure and cholesterol screening, as well as flu shots and mammograms at our larger locations.

Get access to thousands of health and wellness articles, newsletters, tools, and databases on our Web site.

Health and Fitness Centers and Health Club Reimbursement
The Company encourages you to maintain a healthy lifestyle through regular exercise and therefore, has equipped many of our locations with onsite fitness centers. For associates that do not have access to an onsite fitness center, they may be eligible for reimbursement for individual membership dues to a company-recognized health club.

Anthem Rewards
Earn points for every 30 minutes or more per day you participate in healthy activities. Track it online, and redeem your points for Anthem merchandise.

Health Care Flexible Spending Account
This account, which you fund through payroll contributions, allows you to pay for certain health care expenses with pre-tax dollars, and save money by reducing your taxes. Online account access and direct deposit of reimbursements make this benefit even more convenient.

Dependent Care Flexible Spending Account
Pay for daycare or eldercare expenses with pre-tax dollars. Because the amount you contribute is not subject to federal, state, or Social Security (FICA) taxes, you generally see tax savings of 15% to 40%. To help offset the cost, the Company matches 10% of your contribution to the dependent care FSA.
